The following forms are required for registration: To register a vehicle in the state of New York, you will need the two forms that are listed below, as well as three pieces of documentation known as the “3 P’s”: proof of identity (such as a driver’s license), proof of insurance, and proof of ownership (Title to the Vehicle). You will not be able to register your Vehicle until you have all three of these items.

Form MV-82 is necessary for registering most cars in New York.

When registering a car that has just been acquired, you must fill out form DTF-802. This document, known as the Bill of Sale, must be signed by both the person selling the car and the person purchasing it.

This form, ID-82, can assist you in determining which forms of identification you will need to get a registration and title.

OTHER DMV FORMS

  1. DTF-803 When registering your car, you must fill out form to claim a tax exemption.
  2. DTF-804 is the form that must be submitted to claim a tax credit if a vehicle was acquired outside of New York and taxes related to the Vehicle were paid in another state.
  3. ID-44 is a form that will assist you in determining which forms of identification you need to get a driver's license, a learner's permit, or a non-driver ID card.
  4. ID-82 is a form that will assist you in determining which forms of identification you need to get a registration and title.
  5. When applying for a driver's license, you must fill out form MV-44.
  6. This form, MV-44OH, must be completed in order to register a vehicle for historical license plates.
  7. This form, an MV-15, is submitted to the Department of Motor Vehicles to make a record request.
  8. This form, MV-82b, must be completed to register a boat or other kind of watercraft.
  9. MV-95: This form is used to give permission to another individual to register your car under their name.
  10. When a vehicle is sold to a third party, the seller must fill out Form MV-103 to provide details on the Vehicle's mileage and condition.
  11. This application for an In-Transit Permit (MV-82ITP) is the form that must be filled out.
  12. This application is used to request a Salvage Vehicle Inspection and is denoted by the form number MV-83SAL.
  13. This basic information form is issued for pick-up truck owners and is labelled MV-114.
  14. MV-349: This is the form that must be completed to transfer ownership of a vehicle previously registered in the name of a dead individual.
  15. MV-899- This application is used to request a Salvage Vehicle Inspection and is designated.
  16. MV-900- Adding a lien on a vehicle requires the completion of form MV-900, which may be found here.
  17. MV-902- Fill out this application to apply for a duplicate title or to replace a title that has been misplaced.
